چکیده

The optimal catheter ablation approach for longstanding persistent atrial fibrillation (AF) remains elusive despite significant advances made in our understanding of this arrhythmia. A recent systematic review highlighted the significant variation in procedural success rate both within and between techniques, necessitating repeat ablation procedures and anti-arrhythmic drugs to achieve improved outcomes in those with long-standing persistent AF.1 Indeed, current expert consensus statement recommended ablation beyond ostial pulmonary vein isolation for these patients.2 Despite incorporating various substrate modification techniques which commonly include linear lesions and targeting of complex fractionated electrograms, the reported success rates from various laboratories are still below par to those in paroxysmal AF patients.1 Perhaps the often dilated and chronically remodeled atria in longstanding persistent AF patients harbor complex structural substrates capable of maintaining the arrhythmia beyond amelioration even with extensive catheter ablation in some. Certainly, this reflects our incomplete understanding of the complex mechanisms underlying this arrhythmia.

Are the site and mode of termination matters of prime importance in catheter ablation of persistent atrial fibrillation?

Role of AF Termination: Is There Any Difference Between Direct to Sinus Rhythm Versus Via Atrial Tachycardia? During catheter ablation of persistent atrial fibrillation (AF), the arrhythmia often organizes into a regular atrial tachyarrhythmia (AT) or directly terminates to sinus rhythm (SR). The termination of AF during ablation may be associated with higher long-term maintenance of SR, and ha...

Non-Inducibility Or Termination As Endpoints Of Atrial Fibrillation Ablation: What Is The Role?

Catheter ablation is widely used to treat drug-refractory, symptomatic atrial fibrillation (AF). However, beyond pulmonary vein isolation, there remains little consensus on the recommended approach to ablation both in paroxysmal or persistent AF patients. Although ancillary ablation strategies are often used, the lack of a clear endpoint for AF ablation makes it challenging to evaluate their im...

Clinical predictors of termination and clinical outcome of catheter ablation for persistent atrial fibrillation.

OBJECTIVES This study evaluated the role of pre-procedural clinical variables to predict procedural and clinical outcomes of catheter ablation in patients with long-lasting persistent atrial fibrillation (AF). BACKGROUND Catheter ablation of persistent AF remains a challenging task. METHODS Catheter ablation was performed in 90 patients (76 men, age 57 +/- 11 years) with long-lasting persis...

Hierarchical analysis of electrograms to guide termination of persistent atrial fibrillation

The mechanism(s) perpetuating atrial fibrillation (AF) is poorly understood, and long-term success with catheter ablation remains suboptimal, especially in persistent AF. Ablation strategies beyond pulmonary vein (PV) isolation (PVI) have not been shown to provide incremental benefit in terms of maintaining sinus rhythm. However, there are data suggesting that acute termination of AF during abl...

Five-year outcome of catheter ablation of persistent atrial fibrillation using termination of atrial fibrillation as a procedural endpoint.

BACKGROUND This study aimed to determine 5-year efficacy of catheter ablation for persistent atrial fibrillation (AF) using AF termination as a procedural end point. METHODS AND RESULTS One hundred fifty patients (57±10 years) underwent persistent AF ablation using a stepwise ablation approach (pulmonary vein isolation, electrogram-guided, and linear ablation) with the desired procedural end ...
